Our patient list is open, and we welcome new patients. To register permanently with the practice you should be resident within the practice area (see map below).

You will be asked to complete a registration form and a questionnaire for each person registering. The questionnaire allows us to provide medical care in the interim period, while your medical records are transferred from your previous practice. You will also be asked to provide Photo ID and Proof of Address.

Please bring the forms to reception with your photo ID (e.g. photocard driving license or passport) and proof of address (e.g. a recent bank statement or utility bill).
